5Essentials Survey Information

Lake Zurich High School Parents/Guardians and Students,
On March 5, 2025, Lake Zurich High School will take the 5Essentials Survey. This will happen during their 8th-period course.
Please click here to access a Notice for the 5Essentials Survey. This includes information on the survey and also the questions on the survey. It also includes an opt-out section if you do not want your child to participate in the 5Essentials. Please contact Cara Obrochta at cara.obrochta@lz95.org or fill out the opt-out portion in the Notice and turn it into the main office if you do not want your child to participate.
